We are excited to announce that Centre for Quality Improvement and Patient Safety (C-QuIPS) in collaboration with Improving & Driving Excellence Across Sectors (IDEAS), and the support from Health Quality Ontario (HQO) will be the hosting the second Quality Improvement and Patient Safety Forum on October 23, 2017 at the Metro Toronto Convention Centre, South Building.
This targeted event will bring together C-QuIPS members, IDEAS alumni and members of the broader Quality Improvement and Patient Safety community who are actively leading or supporting QI and patient safety practice and academic work.
This year, we are thrilled to have invited Dr. Bryan Sexton, (Director, Duke University Health System Patient Safety Center) as our keynote speaker at the Forum – he will be speaking about resilience in healthcare.
Other highlights of the day will include:
- Hot Topics in Quality Improvement and Patient Safety
- Interactive workshops led by experts in QI and Patient Safety
- Updates about the provincial quality agenda
- Opportunities to network with peers engaged in similar work
Health Quality Ontario’s annual conference, Health Quality Transformation, will be taking place the following day, on Tuesday, October 24, 2017.

The 2017 Quality Improvement and Patient Safety Forum Planning Committee invites you to submit abstracts on:
• A quality improvement project that demonstrates results and/or lessons learned.
• Research focused on patient safety and related outcomes.
Abstracts may include qualitative, quantitative or mixed methods data and must include preliminary or final results in order to be considered for selection. Please refer to the abstract submission guideline document below for more information.
Please email your abstract using the submission template provided below to the Abstract Review Committee at firstname.lastname@example.org no later than August 28, 2017.
The Abstract Review Committee will review all submitted abstracts. The committee will notify authors of the results, via email, by September 18, 2017. If your abstract is accepted, you must confirm acceptance and register for the conference by September 27, 2017. You are responsible for your own travel and accommodation costs.
Note: Only abstracts submitted via the online process will be accepted. You will receive an email confirming the receipt of your submission. If you do not receive an email confirmation within 24 hours of your submission, please email email@example.com.
If you were part of IDEAS cohort 10 and 11, there is no need to submit an abstract for your IDEAS project as part of the open competition. Project teams from these cohorts are guaranteed a poster at this event. For more details, please contact your QI advisor. For all other former IDEAS cohorts, please refer to the submission guideline document for the abstract submission process.
